ALL PLAQUES MUST HAVE A WAIVER FORM TO PROCEED

For Golf Sign Tec to proceed with your order, we MUST have a One-Time-Use Waver signed by a representative of the Club and/or Golf Course that was played to be able to use the logo and course design on your Hole-In-One Plaque before production and/or shipment: No Exceptions.

One-time Waiver Form:

Our One-Time-Use Waver Form is attached to all of our proofs so that you can easily get permission from the golf course to use their logo(s) and courses on your Hole-In-One plaque(s). We do this as both a protection for ourselves as well as you as a player. We ask that you have the Waver Form returned to us at the same time you approve the graphic so that we can continue to provide a smooth and expedient production time.
