Next to your preferred wood choice below, please enter all of the golfers pertinent information in the form before adding your plaque to the cart. To be able to create an accurate representation of the hole that was played, we will need the exact website where that hole can be found. If you would like the logo from the Country Club or Course where this Hole-In-One was played, you will need to obtain a VECTORIZED GRAPHIC FILE for us to use and fill out the Course Waver. You can obtain this graphic file from the club or course where you played. If one is not available, we can carve a simple course name in the logo area.
Once you have placed your order with all of the above available information, we will send you a graphic proof for your approval at the email address you provide. We do not carve until we have your 100% approval and the Course waver stated above. Please check this approval form for errors in spelling and hole specifics. Once approved, we will start the carving process and will ship this out in about four weeks from the approval date.
